I found this booklet from our local healthcare system dated 2015 in my files, acquired sometime after my husband's stroke. I'm finding it another good resource to help me with my "Wishes" and how to share them with my Health Care Agent and family, friends. it's another planning resource; does not replace Five Wishes.

The Personal Decision booklet was provided by the Deerfield Fire dept with instructions to keep it in a plastic bag on your refrigerator with blue dot they provide on your front door
Also put medical info, id and insurance cards, emergency contact info, hospice info, & memorial/funeral arrangements in bag. Nice to have all those goodies in one spot even if your fire dept. isn't looking for it
